The Home Health Clinician Case Manager, RN cares for a group of patients consistently using the nursing process and clinical standards and protocols.

The Home Health Case Manager, RN responsibilities include:

* Managing the activities of the multi-disciplinary team providing care to the patient
* Monitoring the provision of therapeutic services through written and verbal communication with all disciplines
* Ensures that all visits scheduled and provided are authorized and covered by physician orders
* Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
* Informs team assistant of any schedule changes and communicates caseload and patient care issues to Patient Care Supervisor

Home Health Case Manager, RN specific skills:

* Knowledge of regulatory standards appropriate to position
* Written and verbal communication skills
* Computer skills appropriate to position

Certifications and Licensures:

* Required BLS (Basic Life Support)
* Required RN (Registered Nurse) License
* Required Driver's License State of Florida

Education:

* Required Associate's or Diploma Nursing
* Preferred Bachelor's Nursing

Experience:

* Required 1 year Registered Nurse or 1 year Licensed Practical Nurse
* Preferred 1 year Home Care
