Job Description Summary:

The role of the Intake Coordinator is one that requires a great deal of compassion and patience. They are responsible for interacting with prospective clients who are seeking care, as well as their families and loved ones. In this role, they must be able to listen carefully to the needs of these individuals while also ensuring that all relevant information is properly documented in an organized fashion.

This position's primary responsibilities include executing the full intake process for all prospective new clients. The individual in this role will become an expert in BrightStar Care's service offerings and provide appropriate service solutions and customer service to our prospective clients. The Intake Coordinator will follow a client from the initial inquiry call through enrollment in services. This is primarily an in-office position, Monday through Friday, 8A-5P, with some evening and weekend commitments required.

Responsibilities:

The Intake Coordinator's primary responsibilities are:

* Gather information about the prospective client's health status, living situation, social support system, and goals for care.
* Answer clinical questions and provide clarification on any questions about home health aide tasks/scope of practice.
* Be an expert in home care and general health care industry knowledge, providing appropriate solutions and referrals for solutions outside of our service offerings.
* Schedule in-person consultations to be completed by BrightStar Care's Registered Nurse.
* Determine appropriate bill rates for services.
* Complete data entry in BrightStar Care's ABS to include an intake form, the client's demographic information, call notes, follow-up tasks, and reminders.
* For clients with an imminent need, and for which an in-person consultation may not be feasible, complete enrollment of care, to include service agreements and payment information, and coordination with BrightStar Care's operations team.
* Coordinate with BrightStar Care's referral partners to ensure a seamless transition of care.
* Complete short and long-term nurturing of leads.
* Attend community and marketing events as directed.
* Participate in the Sales departments' on-call rotation
Qualifications:

* High school diploma or higher
* Experienced Home Health Intake Coordinator preferred but not required
* Experience medical office knowledge preferred
* Must be able to pass a Level 2 Background Check.
* Knowledge of Medicare regulations preferred but not required
* Excellent interpersonal skills and ability to communicate verbally and in writing effectively.
* Demonstrates proven decision-making skills.
* Knowledge and the ability to apply understanding of medical terminology.
* Ability to multitask, ensuring the completion of many tasks throughout the day.
* Basic, functional knowledge of computer/technology skills including, but not limited to MS Office, MS Excel, and electronic medical record with good typing skills.
* Ability to exercise independent judgment, work well with marketing team, office personnel and hospital and doctor offices and communicates with referral sources confidently and knowledgeably.
Environment, Personal Protective Equipment or Physical Demands

Environment: Office Position with contact with clients under a wide variety of circumstances.

Physical Requirements: Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the ability of the client. Must be able to lift up to 50 pounds. Visual and hearing acuity required.
